As he acknowledges in the responses, the PSLF program was signed into law by President George W. Bush, so maybe he deserves the thanks. But, the program has been a disaster, with most of the applications for loan forgiveness being denied. And, it does seem wild that you could be a salaried, hospital employee (as he also states in the replies) of one of the biggest, best, and richest hospitals in America, in one of America's most expensive cities, that is not lacking for doctors, and still qualify as a borrowers in public service, simply because your employer is a not for profit. You're right, which means the good doctor is some combination of ignorant and dishonest. He's probably been on some income-based forbearance at least some significant time between when he graduated from med school and now (residencies, fellowship training, etc.). Thus, his balance due when he started paying was almost certainly far more than it was when he graduated.
He could have re-financed (and/or consolidated) somewhere in there, too. Those fees can be steep, and they're typically folded into the principal.

People can be really dumb about finance. There was an assistant professor I worked with at my first tenure-track gig (in a college of business!) who'd re-financed his mortgage and was bummed that they'd not started his payment schedule such that he'd get a "free" month. Dude, there ain't no "free" months ... you might go a month without making an actual payment now, but you're either paying a tiny bit more monthly or you're paying more in prepaid interest at closing.
At least the fool wasn't trumpeting his ignorance all over the internet.
